Hybrid RN Case Manager - Telehealth & In-Home Care (IL)

Ponos Care in Chicago, IL is seeking an RN Case Manager to support Medicaid populations through hybrid telehealth and in-home visits. This role integrates telephonic case management, TOC, RPM and LTSS responsibilities across multiple states to promote independence and quality of life.
