Job Description

We are looking for a Registered Nurse with home health experience to join our team and provide quality nursing support to clients in their homes.

Responsibilities:

Provide nursing care and support to clients in a home care setting.
Assess client needs and monitor changes in condition.
Develop and follow appropriate care plans based on client needs.
Communicate with clients, families, caregivers, and the care team regarding client care.
Complete accurate and timely clinical documentation.
Monitor and report changes in client condition to the appropriate care team.
Coordinate with caregivers and other healthcare professionals as needed.
Follow applicable clinical, safety, and documentation requirements.
Maintain client confidentiality and professional standards at all times.

Qualifications:

Current and valid Registered Nurse (RN) license.
Previous home health experience required.
Strong clinical assessment and communication skills.
Ability to work independently in a home care environment.
Strong documentation and organizational skills.
Ability to communicate effectively with clients, families, caregivers, and healthcare professionals.
Reliable and professional approach to client care.

Preferred:

Experience working with home care or home health clients.
Familiarity with care plans, clinical documentation, and coordination of in-home services.
