We’re seeking a dedicated Rhode Island Licensed Physical Therapist to provide one-on-one care to patients in their homes. If you’re passionate about helping patients regain strength and independence while enjoying a supportive, collaborative environment — we’d love to meet you! What You’ll Do Provide comprehensive physical therapy evaluations and individualized treatment plans. Deliver high-quality care and patient education in the homecare setting. Communicate effectively with physicians, caregivers, and the rehab team to ensure seamless care. Participate in case conferences, admissions, and discharge planning. Complete documentation accurately and in compliance with agency standards. Assess and recommend adaptive equipment as needed. Supervise and instruct health team personnel as appropriate.
