Nurse Practitioner OR Physician Assistant - Onsite PRN

Position Summary Concentra is looking for a PRN (as-needed) Advanced Practitioner for our onsite clinic at our client's site in Sacramento, CA. Clinic hours (as‑needed coverage opportunities) M/W/F 6:00am-2:00pm, T/Th 10:00am-6:00pm The Onsite Nurse Practitioner works with the Onsite Leadership Team at the assigned onsite clinic, and under the direction of the Supervising physician or Director of Medical Operations to implement the location’s medical policies, procedures, and programs ensuring the delivery of efficient, responsible and accountable healthcare of the highest quality to patients, businesses, and industry in accordance with Concentra's Best in Class practice. Essential Duties and Responsibilities Spends 100% of time in the onsite clinic providing direct patient care, leading by example, and creating an exceptional patient experience. Ensures day-to-day execution of Concentra's clinical model collaborating with members of the healthcare team to drive optimal clinical outcomes. Accountable for monitoring, understanding, and effecting key onsite clinic metrics that relate to quality outcomes. Advanced Primary Care - chronic condition management, gaps in care, preventive health and age‑appropriate screenings, vaccinations, SDOH, wellness initiatives, clinic utilization and patient satisfaction. Occupational Medicine - timely return to work measures after an injury on the job, case duration, lost time, work duty status, reduction in work‑related illnesses and improved functional capacity. Provides direct patient care through compilation of the medical history, conduction of the physical exam, diagnosis, and the development of appropriate treatment plans. Administers or orders diagnostic tests, such as x‑ray, electrocardiogram, and laboratory tests. Maintains timely, accurate and detailed patient records and paperwork. Interprets diagnostic test results for deviations from normal. Performs therapeutic procedures, such as injections, immunizations, suturing and wound care, and managing infection. Instructs and counsels patients regarding compliance with prescribed therapeutic regimens and health maintenance. Listens to and understands internal and external client needs. Assists in the development, implementation and revision of policies affecting medical practice judgment and quality of clinical care. Assists in the development and presentation of education and instructional programs. Provides excellent and compassionate customer service to all patients, clients and peers while incorporating Orange Book values. Education/Credentials Education Level: Bachelor’s Degree Major: Nursing or related health field Degree must be from an accredited college or university. Education Details: Bachelor’s degree from an accredited college or university or equivalent education and experience Master’s degree in nursing or related health field from an accredited college or university Graduate of an accredited Nurse Practitioner program Certifications and/or Licenses Current licensed nurse practitioner in the state where employed and in accordance with state laws of practice Must maintain work state nurse practitioner licensure throughout the course of employment Job-Related Experience Customarily has at least the following experience: 3 years Customarily has at least three years of demonstrated experience in occupational medicine, urgent care or an emergency setting Job-Related Skills/Competencies Concentra Core Competencies of Service Mentality, Attention to Detail, Sense of Urgency, Initiative and Flexibility Ability to make decisions or solve problems by using logic to identify key facts, explore alternatives, and propose quality solutions Outstanding customer service skills as well as the ability to deal with people in a manner which shows tact and professionalism The ability to properly handle sensitive and confidential information (including HIPAA and PHI) in accordance with federal and state laws and company policies Demonstrated knowledge of occupational medicine requirements (state specific) Demonstrated working knowledge of clinical operations Knowledge of laws and regulations that govern delivery of rehabilitation services Knowledge of worker’s compensation and clinical procedures and all processes involved in the delivery of quality care.
