Casual Clinical Transplant Coordinator II
UPMC Hamot is hiring a casual Clinical Transplant Coordinator II to join the team!
Purpose: The Clinical Transplant/VAD Nurse Coordinator II has the authority, responsibility, and accountability for the delivery of transplant/VAD nursing care through the use of the transplant/VAD nursing process. The Clinical Transplant Coordinator II, in collaboration with the Supervisor, Clinical Transplant/VAD Coordinators, transplant/VAD team and other health care professionals, applies competent clinical knowledge and skills to achieve quality care outcomes. The Clinical Transplant/VAD Coordinator II must demonstrate the knowledge and skills necessary to provide care that is relevant to patients under their care, as well as apply principles of growth and development over the life span. The Clinical Transplant/VAD Coordinator II also must possess the ability to assess and interpret patient data needed to identify each patient's requirements relative to his or her age specific needs.
Responsibilities:
- Coordinate transplant/VAD referrals, evaluate candidate eligibility, identify contraindications, and manage the multidisciplinary evaluation process.
- Advocate for patients and families by addressing educational, developmental, cultural, and support needs throughout the transplant journey.
- Ensure compliance with UNOS regulations, maintain listing status, and submit required data to regulatory agencies.
- Build and maintain relationships with referring physicians and healthcare providers to support patient care and strengthen referral networks.
- Provide education and outreach to patients, families, healthcare professionals, and community organizations regarding transplant/VAD services.
- Maintain ABTC certification, stay current on transplant/VAD research and best practices, and participate in professional organizations and conferences.
- Mentor and train staff, support clinical research initiatives, and serve as a resource for colleagues and interdisciplinary teams.
- Participate in leadership activities including hiring, orientation, staff development, committee involvement, and clinic oversight.
- Develop and implement patient education plans, coordinate ongoing assessments, and maintain comprehensive documentation before transplant/VAD procedures.
- Facilitate transplant/VAD admissions, review recipient eligibility, coordinate with organ procurement agencies, and support peri-operative care.
- Monitor inpatient recovery, educate recipients and families, collaborate on care plans, and communicate patient status to providers and payors.
- Manage post-transplant/VAD outpatient care by monitoring organ function, coordinating follow-up services, adjusting therapies as ordered, and supporting long-term treatment adherence.
- Identify and address complications, ensure continuity of care, reinforce wellness and follow-up requirements, and manage complex patient cases.
- Demonstrate continuous professional growth through feedback, self-development, project participation, problem-solving, and on-call responsibilities.
3 to 5 years of clinical nursing experience required.
2 to 3 years of recent transplant/VAD coordinator experience (within 5 years of hire) required.
Demonstrated expertise in professional transplant/VAD nursing practice.
Diploma or Associate Degree in Nursing required.
BSN preferred.
Must maintain a Solid/Strong/Good or higher annual performance rating to retain Clinical Transplant Coordinator II status.
Must achieve a Solid/Strong/Good or higher performance rating to be eligible for promotion to Supervisor, Clinical Transplant Coordinator.
Licensure, Certifications, and Clearances:
- Certified Clinical Transplant Coordinator OR Certified Clinical Transplant Nurse
- Registered Nurse (RN)
- Act 34
- Current Pennsylvania licensure as a Registered Professional Nurse. Certification from the American Board for Transplant Certification (ABTC) as a Certified Clinical Transplant Coordinator (CCTC), CHFN (Certified Heart Failure Nurse), Certified Clinical Transplant Nurse (CCTN), or other UPMC approved certification associated with the service line required at time of hire.
Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
