Home Health Physical Therapist

Evaluates, plans, and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint, and bone diseases. Aims to improve or restore function, increase strength, relieve pain, and enhance functional independence. Demonstrates competencies specific to the population served. Interprets and implements referrals. Evaluates patient functional abilities and disabilities such as range of motion, strength, and coordination through standardized assessment and testing. Establishes functional baseline to be used throughout treatment as a gauge of progress and physical improvements. Engages in interdisciplinary discussion and rounds. Establishes education and treatment planning. Participates in family and community education. Discusses patient care goals and estimates length of treatment with the interdisciplinary team. Provides direct patient care, conducting individual and/or group physical therapy activities according to the treatment plan. Facilitates and instructs patients on skills necessary to participate successfully. May design and fabricate splints and other assistive devices. Participates in program development and department process improvement. Serves as a resource person in areas of expertise. May provide training in areas of expertise for staff. May need to cross-train and provide coverage in other departments as needed. Documents evaluations, patient treatment plans, treatments performed, and progress. Indicates conditions in relation to established goals and makes alterations to treatment plans to enhance therapy potential. Maintains accurate, legible, and complete records for each patient served. Responsible for timely documentation and communication of any limitations to leadership.
