One Love Agency is hiring a full-time Behavioral Health Case Manager for our Pocatello office, starting immediately.

Who we are

For more than fifteen years we have walked with families through the hardest seasons of their lives. Six offices now, across Idaho and Utah. Adults in crisis. Kids and families who need someone in their corner. People finding their way back from addiction. We serve the folks other systems have the hardest time holding onto.

We run on a Hawaiian word: hānai — to take in, to raise as your own. That is how we treat our clients and it is how we treat each other. When someone joins One Love, they join a family.

We also work like a village. The case manager, the CBRS specialist, the therapist, the peer support specialist, the front desk, the biller — every one of us is standing in a circle around a client. Nobody is the star and nobody is support staff. If the front desk misses a benefit check, the best hour you ever spent with a client never gets to count. We each carry a piece, and we carry it for each other.

And we hold to three things we call the Big C's :

Clients. They are the reason for every decision we make. Not the schedule, not the paperwork, not convenience. Them.

Compliance. A note written well is proof a person was actually served. It is how we protect a client's access to care, protect the staff who provide it, and protect the agency that employs them. We stay at 100% — not because someone is watching, but because the people we serve cannot afford for us to slip.

Community Partners. Schools, courts, the hospital, Health and Welfare, recovery programs, housing and benefits offices. They send us people they are responsible for, and that trust is on loan. We do what we said we would do, and when we can't, we tell them the truth early.

The work

You will carry a case management caseload serving both adults and youth in the Portneuf Valley — people with serious mental illness, co-occurring substance use, and kids and families in the YES system.

Day to day that looks like: assessing strengths, needs, and goals alongside the client and the people who matter to them; building the plan of service and keeping it alive; connecting and coordinating the pieces — medical and behavioral health care, school supports, housing, benefits, employment, transportation, natural supports; advocating when a system will not move; and tracking whether life is actually getting better, then changing the plan when it isn't.

You will work under supervisory protocol with our licensed clinicians, alongside our CBRS specialists, therapists, and peer support staff, and you will report to our Pocatello Director with a full administrative team behind you.

And you will carry the documentation — timely, tied to the plan and the CDA, and built to hold up under a Medicaid or IBHP review.

Here is the part we want to be plain about

We are not looking for someone to fill a slot. We are looking for a case manager who has done this work, looks at a caseload and this community and says I've got this — and then owns it. Someone who already knows the Pocatello resource map because they have actually worked it. Someone whose name means something to the schools, the courts, and the clinics. Someone who will grow what we can do here because they see the people we are not reaching yet.

Serve the client. Own the caseload. Take care of our partners. That is the job.

What we are looking for

Honestly? Someone who gets it, wants it, and can carry it.

Required

• Bachelor's degree in a health and human services field — social work, psychology, counseling, sociology, human services, or a related field — meeting Idaho Behavioral Health Plan case management qualifications and able to practice under supervisory protocol
• Licensed candidates — BSW, MSW, LSW, LMSW, LCSW, LPC, LCPC, LMFT, or RN — are strongly encouraged to apply and may qualify for expanded scope
• Ability to pass an Idaho Department of Health and Welfare criminal history background check and be cleared before direct client contact
• Clear of any state or federal exclusion list (OIG LEIE / SAM) — we verify at hire and monitor on an ongoing basis, as Medicaid participation requires
• Valid Idaho driver's license, reliable transportation, and current auto insurance
• Comfort with electronic health records and a real commitment to same-week documentation

What makes you stand out

• You have done this work. Case management, CBRS, YES, or equivalent community-based behavioral health experience — you are not learning the job on our clients.
• You know this valley. You have a working map of Bannock County's resources — schools, housing, benefits, treatment, recovery, food, transportation, legal, medical — and standing with the people who run them. When you make a call, people pick up.
• You know how the system moves. Eligibility, CDAs, plans of service, units and authorizations, medical necessity, IBHP expectations. You see the compliance issue before it becomes a recoupment.
• You are solid on documentation. Your notes are on time, tied to the plan, and hold up under review — because you understand that clean charting is how a client keeps their access to care.
• CANS certification (Idaho YES) for the youth side of the caseload
• Experience with adults with serious mental illness, co-occurring substance use, or developmental disabilities
• Bilingual or multilingual ability — a genuine gift in this role, though not required
• Trauma-informed, culturally humble, and steady when a day goes sideways

The practical details

Full-time, hourly, based in our Pocatello office, Monday through Friday, with community-based work as the client's plan requires. Competitive wage based on your education, licensure, and experience. Benefits available. Immediate start.
