Greenfield Rehabilitation Agency is excited to announce an opening for a PRN Physical Therapist (PT) to join our outstanding therapy team in Elkhorn, Wisconsin! What You'll Do:
- Complete functional mobility and physical performance assessments
- Create individualized treatment plans tailored to resident needs
- Provide skilled PT interventions that improve mobility, strength, and quality of life
- Educate residents, families, and caregivers on fall prevention and safe mobility
- Recommend assistive devices and functional adaptations
- Collaborate with an interdisciplinary team to support whole-person care
- Maintain accurate clinical documentation
Qualifications:
- Graduate of an accredited PT program (Bachelor's, Master's, or Doctorate)
- Licensed or eligible for Wisconsin PT licensure
- Strong clinical judgment, communication skills, and professionalism
- A passion for improving the lives of older adults
