Greenfield Rehabilitation Agency is seeking compassionate, motivated PRN Physical Therapists to support our well-respected, resident-centered senior community in Delavan, Wisconsin.

What You'll Do as a PRN PT
- Perform functional mobility, balance, and strength assessments
- Develop and implement individualized, goal-driven treatment plans
- Provide skilled PT interventions to improve mobility, safety, and independence
- Educate residents and caregivers on mobility strategies and fall prevention
- Recommend appropriate assistive devices and environmental modifications
- Collaborate with an interdisciplinary care team
- Complete timely and accurate clinical documentation
What You Bring
- Graduate of an accredited Physical Therapy program
- Current or eligible Wisconsin PT license
- Strong clinical reasoning and communication skills
- Passion for working with older adults with dignity and compassion
- Flexibility and a collaborative mindset
