Responsibilities: Conduct comprehensive in-home patient assessments to determine care needs and eligibility Develop and document individualized care plans based on assessment findings Evaluate patient health status, functional abilities, and home environment safety Collaborate with physicians, care teams, and family members to coordinate appropriate services Ensure accurate and timely clinical documentation in compliance with regulatory standards Monitor and reassess patient conditions, adjusting care plans as needed Serve as a patient advocate, connecting clients with community resources and support services Requirements: Active and unrestricted Registered Nurse (RN) license in the state of Florida Prior experience in home health, case management, or community health nursing preferred Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills with patients and families Ability to work independently and manage a flexible schedule in the field Reliable transportation and valid driver's license required Proficiency with electronic health records (EHR) and clinical documentation About Us: ComForCare Home Health Care has been a trusted name in compassionate, client-centered home care services, helping individuals live their best lives at home.
