Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ) is seeking a field-based Dual Special Needs Case Manager to assess and coordinate care for members using established guidelines, ensuring appropriate and cost-effective services. You will collaborate with clinicians, families, and providers to implement a member-centric plan of care across the continuum.
