Heartland Hospice, part of Gentiva, is seeking an Afterhours RN Care Manager to provide compassionate hospice care outside standard hours. You will ensure seamless coverage, maintain clinical oversight, and support patients and families during nights, weekends, and on-call periods. The role involves direct patient visits, assessments, and coordination with the Interdisciplinary Group (IDG). You will educate families, document care plans, and help drive quality and compliance while leading field #J-18808-Ljbffr
