Collaborate with the care team to coordinate therapy plans and discharge recommendations. Work 40 hours per week with 8-hour shifts. Evaluate functional status and develop a treatment plan aligned with clinician goals and patient needs. Perform reassessments and update the plan of care based on patient response.
