Certified Nurse Assistant (CNA) - Long Term Care (LTC)

Job Description

The Certified Nurse Assistant (CNA) will provide essential supportive services to nursing staff and assist residents in activities of daily living, ensuring adherence to care plans and established policies and procedures.

Key Responsibilities

Assist residents with activities of daily living, including lifting, turning, moving, and transporting into and out of beds, chairs, and wheelchairs.
Coordinate dining room services during assigned mealtimes, which includes set-up and clean-up, meal tray delivery, and providing feeding assistance. Document meal intake accordingly.
Deliver nutritional supplements to residents at assigned times and offer assistance as necessary; document intake accurately.
Assist with weighing residents as per facility policy and report any weight changes to nursing staff.
Monitor the condition of residents' skin and report any concerns such as pressure areas or skin breakdown to nursing staff.
Support nursing staff in implementing toileting programs for residents.
Complete flow sheets daily to document executed tasks and resident progress as directed.
Communicate factors that may hinder the performance of assigned responsibilities, such as resident refusals or reassignment to another unit.
Report any changes in residents' conditions to the appropriate nursing staff and supervisors.

Requirements

Minimum of 1 year of experience in long-term care.
High school diploma or equivalent.
Ability and willingness to travel as required.
Current BLS certification from AHA.
Proven work history showing employment of at least 13 weeks in previous positions.
No disciplinary actions in effect against certification.

Weekend and Shift Requirements

Shift: 2 PM - 10 PM on all weekends.
Must be willing to float to other units as needed; blocked scheduling will not be approved.

Documents Required

Skills checklist specific to the discipline completed within the last year.
Two supervisory references from the last two years of employment.
Active license or certification at the time of submission.
