About the Role:
We are seeking a compassionate and skilled Home Health Caregiver to join our team of caregivers. As a Home Health Caregiver, you will play a vital role in providing one-on-one care to our clients in the comfort of their own homes. Your responsibilities will include:
Providing assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
Assisting with medication management
Maintaining a clean and safe living environment
Encouraging independence and supporting clients in their daily routines
Communicating effectively with clients, families, and other healthcare professionals
Developing and implementing personalized care plans in collaboration with our interdisciplinary team
Responsibilities:
Provide high-quality, patient-centered care to clients in their homes
Collaborate with our interdisciplinary team to develop and implement personalized care plans
Maintain accurate and detailed records of client care
Participate in ongoing training and education to stay current with industry best practices
