Job Description Job Description Home Health Intake Referral Coordinator The Home Health Intake Coordinator manages the initial patient referral process, serving as the key liaison between hospitals, physicians, families, and clinical staff to ensure a smooth start of care. This role is responsible for receiving referrals, verifying insurance benefits, obtaining authorizations, and accurately entering patient information into the EMR system. As an Intake Referral Coordinator, you will: Receive and appropriately document new referrals in a timely manner Verify patient payor sources and maintain insurance authorizations and initial certifications Coordinate care with Nursing Supervisors, patients, and the interdisciplinary team Communicate effectively with patients, staff members, healthcare professionals, and referral sources Respond to agency and patient needs in a professional and solutions-focused manner Demonstrate commitment to professional growth and competency Participate in agency-sponsored in-service training Support the Agency’s QAPI program and quality initiatives Promote agency philosophy and administrative policies to ensure quality patient care Requirements Minimum one year of referral intake experience preferred Experience with third-party payers, Medicare, and Medicaid highly preferred Medical field background preferred, including knowledge of medical terminology Strong computer and customer service skills required LVN/LPN preferred Valid Driver’s License and current Auto Insurance required Homecare Homebase experience preferred Benefits & Perks Medical, Dental, and Vision Insurance 401(k): Eligible after 3 months of service and fully vested immediately Choice Health At Home matches 100% of the first 3% contributed each pay period Health Savings Account (HSA) Life Insurance Short-Term and Long-Term Disability Insurance Paid Time Off and Paid Holidays
