As a Physical Therapist with Elite, you’ll play a key role in "Keeping Our Community Home," helping patients achieve their mobility goals and regain strength while fostering a supportive, healing environment. Job Summary The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. Duties & Responsibilities Assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Provide patient, caregiver, and family counseling. Provide patient and caregiver education. Prepare clinical notes and progress summaries based on attainment of goals. Communicate with the physician responsible for the home health plan of care and other healthcare practitioners (as appropriate) related to the current home health plan of care. Provide physical therapy services to patients according to a written physician’s plan of care. These may include, but will not be limited to: Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care. Evaluate the home environment. Assess muscle strength, mobility, gait, ROM—potential for rehab. Direct physical therapy treatment. Instruct patients and families/caregivers in the use and care of therapeutic appliances. Determine priority needs for physical therapy. Report to physician patient’s reaction to treatment or changes in condition. Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of therapy and their goals through case conferences. May train patient in use of prosthetic device. Identify patient and family/caregiver needs for other home health services and refer as necessary. Participate in discharge planning for patient. Provide in‑service education programs for nursing organization personnel as needed. Participate in peer consultation process. When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks. Supervise Physical Therapy Assistants according to organization policy and state regulations. Participate in the HHA's quality assessment and performance improvement program and HHA-sponsored in‑service training. Job Requirements Licensed to practice as a physical therapist within the State of Idaho and the State of Washington. Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master’s program in physical therapy approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or any successor organization. Two years of appropriate experience as a physical therapist; community/home health experience preferred. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Must be a licensed driver with a vehicle insured per state or organization requirements and in good working order.
