Charge Nurse - LVN

The Charge Nurse - LVN/LPN supervises the day-to-day nursing activities of facility or unit within facility in accordance with federal and state regulation, Company policies and procedures and nursing standards so as to provide the highest quality of nursing care to our residents.

Duties and responsibilities include:

• Directing and supervising day-to-day functions of assigned personnel and ensuring that policies and procedures are followed
• Overseeing implementation of resident plan of care and evaluating resident response
• Completing routine rounds during shift to observe residents, to determine if nursing needs are being met and to ensure nursing personnel are performing work assignments in accordance with acceptable nursing standards, facility policies and procedures, and governmental regulations
• Initiating emergency support measures as needed
• Monitoring treatment schedules to ensure they are provided as scheduled and may perform duties of Treatment Nurse
• Completing requisitions and arranging for diagnostic and therapeutic services, as ordered by physician, and in accordance with facility's established procedures
• Contributing to formulation/review of nursing care plans weekly and PRN
• Providing resident/caregiver instruction and education as necessary
• Encouraging independence by resident in activities of daily living to fullest extent possible
• Receiving and giving change of shift report upon reporting in and ending shift
• Performing documentation responsibilities in accordance with company requirements
• Preparing and administering medications as ordered by physician in accordance with governmental regulations and facility policies and procedures
• Coordinating with resident's physician in the care of resident including notification upon change of condition
• Making rounds with physicians as necessary
• Identifying resident safety concerns and taking appropriate actions to maintain a safe environment
• Performing other related duties as required

Knowledge, skills, abilities, and qualifications include:

• Nursing degree from accredited college or graduation from approved LVN program
• LPN/LVN license required
• Valid Adult Basic First Aid and CPR Certification preferred
• A minimum of 1-3 years of experience in direct patient care, preferably in acute or long-term care setting, with responsibility for providing nursing care to multiple residents preferred; previous Charge Nurse or supervisory experience preferred
