Please review the job description below and contact me ASAP if you are interested.Job ID:26-21363 Pay Range: Regular Pay: $42/hr Weekly Housing & Meal Stipend: $1680/weekly Weekly Per Diem: $1021.72 Weekly Gross Pay: $ 2,701.72 (subject to completion of scheduled hours)Key Responsibilities:Coordinate and manage patient care from admission through discharge or transfer.Assess, plan, implement, and evaluate patient care using the nursing process.Collaborate with physicians, nurses, therapists, patients, families, and insurance providers to develop and coordinate individualized care plans.Review admissions, discharge planning, observation stays, readmissions, and utilization management to ensure appropriate level of care.Initiate and monitor clinical pathways based on admission diagnoses.Advocate for patients and families while facilitating communication among the healthcare team.Evaluate quality and utilization cases to improve patient outcomes and resource management.Maintain accurate documentation using EPIC EMR.Support patient transitions to appropriate levels of care and community resources.May provide case management support in the Emergency Department as assigned.Contract Length: 13 WeeksShift: 8-hour shifts, 8am - 4:30pm, weekends and weekdaysScheduled Hours: 40 hours [Hours per Facility]Key Requirements and Technology Experience:Active California Registered Nurse (RN) License.Minimum of 1 year of recent Case Management experience in an acute care setting.Current BLS/CPR Certification.Strong knowledge of discharge planning, utilization review, and care coordination.Experience with EPIC EMR preferred.EMR: EPICBilingual: Not requiredHoliday Coverage: Not requiredOur client is a leading Healthcare Industry and we are currently interviewing to fill this and other similar contract positions.
