The RN Care Manager at Life Care Centers of America coordinates the patient’s care and services, promoting quality care through collaboration with patients, families, care teams, payers, and external entities to ensure safe transitions from the facility to the next setting. Applicants should hold a nursing diploma and an active RN license; CCM or RN-BC is preferred.
