Spiritual Care Associate (Labor Pool)

Overview The Spiritual Care Associate provides emotional and spiritual support to patients, families, and caregivers. As a member of the interdisciplinary team, you support the mission and philosophy of Riverside Hospice Agencies and collaborate with local clergy and spiritual coordinators as needed. Responsibilities Conduct spiritual assessments promptly after admission and support patients in accordance with ongoing wishes and needs. Develop individualized spiritual/pastoral care plans and collaborate closely with clergy and team members to meet patient and family goals. Recognize and respond to changes in patient condition; communicate changes to physicians and team in a timely manner using current processes and technology. Document counseling and spiritual support provided in the patient’s clinical record. Complete required continuing education classes and in-service programs applicable to home care in a timely manner and stay current with clinical knowledge. Participate in interdisciplinary team meetings and discussions focusing on quality and outcomes. Delegate and prioritize tasks to facilitate quality patient care and ensure all patient care needs are met. Achieve satisfactory individual productivity measures based on FTE/Status and average number of assigned patients in case load. Build trusting collaborative relationships with patients, families, and team members. Participate positively in training and support of all team members. Adhere to CHAP standards and departmental policy when notifying physicians and team. Work call rotation, weekend shifts, and float in more than one work location. Qualifications Associate’s Degree (Required). Theology (Preferred). 1 unit of Pastoral Care Education (CPE) (Preferred). Valid driver’s license (Required). Own personal vehicle and drive Right Hand Side business for 25% or more of the time to perform essential functions of the job. #J-18808-Ljbffr
