Summary: As a Hospital Liaison, you will be responsible for building and maintaining strong relationships with local hospitals, communicating effectively with healthcare providers, and facilitating the transition of patients from hospital to home or rehabilitation. With a focus on patient-centered care, you will work closely with our interdisciplinary team to ensure a smooth and efficient discharge process. Responsibilities: Establish and maintain relationships with local hospitals, physicians, and other healthcare providers Coordinate patient transfers from hospital to home or rehabilitation, ensuring seamless communication and timely discharges Facilitate the sharing of patient information between healthcare providers, ensuring accurate and timely communication Collaborate with the healthcare team to develop and implement plans of care that meet the unique needs of each patient Monitor and report on patient outcomes, identifying areas for quality improvement Participate in quality improvement initiatives and contribute to the development of policies and procedures Maintain accurate records and reports, ensuring compliance with regulatory requirements Requirements: Bachelor's degree in Healthcare Administration, Nursing, or related field At least 3 years of experience in medical administration, hospital liaison, or a related field Strong communication and interpersonal skills, with the ability to build and maintain relationships with healthcare providers Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple projects simultaneously Strong problem-solving skills, with the ability to think critically and creatively Ability to maintain confidentiality and handle sensitive information with discretion If you're passionate about delivering exceptional patient care and have a strong background in medical administration, we encourage you to apply for this exciting opportunity. #J-18808-Ljbffr Hallmark Health Consulting Services
