We are looking for licensed Physical Therapists (PT), Physical Therapy Assistants (PTA), Occupational Therapists (OT), and Certified Occupational Therapy Assistants (COTA) to join our growing network in Yuma County. What You'll Do Provide Skilled Home Care: Deliver individualized physical or occupational therapy services (evaluations, treatments, or assistant-level plans of care) in the patient's home environment. Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction to help patients reach their independence goals. Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain seamless communication between therapists and assistants to ensure a coordinated approach. Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care. Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).
