Lead Outreach Navigator for Homeless Services

Community Bridges, Inc. is hiring a Lead Navigator to support individuals experiencing homelessness in Arizona. The role focuses on outreach, engagement, case management, and wrap-around services within a multi-disciplinary team. The Lead Navigator mentors Navigator II, coordinates care with RBHA and community partners, and ensures high-acuity cases receive appropriate services.
