Pay Rate: $2,650 – $2,729/Week

Position Overview

We are seeking an experienced RN Case Manager to coordinate patient care, support discharge planning, and facilitate transitions of care in an acute hospital setting.

Required Qualifications

• Active Alaska RN License in hand required for Auto Offer.

• BLS certification.

• Minimum 1 year of experience.

• Acute Care experience required.

• Case Management experience required.

• Epic experience required.

• First-time travelers accepted.

• Must be willing to work weekends, including every other weekend or 4 weekend days within a 4-week schedule.

• Must be willing to float within scope to meet facility needs.

• ACM Certification preferred.

Responsibilities

• Provide RN case management services in an acute hospital setting.

• Coordinate patient care and appropriate healthcare services.

• Assess patient needs and support discharge planning.

• Collaborate with physicians, nurses, social workers, and other interdisciplinary team members.

• Review and document patient care plans and case management activities.

• Coordinate transitions of care and appropriate post-acute services.

• Maintain accurate documentation in Epic.

• Support quality, safety, and compliance standards.

• Float within scope as needed to meet facility needs.
