We are seeking an experienced LICSW Social Worker for an inpatient hospital setting in Massachusetts. This role focuses on providing social work services to patients and families, coordinating discharge planning and after-care services, and collaborating with community agencies and interdisciplinary healthcare teams. 

Key Responsibilities 
Provide psychosocial support and assessments for patients and families 
Coordinate discharge planning and after-care services 
Maintain communication with referral agencies and community resources 
Collaborate with physicians, nurses, and interdisciplinary care teams 
Manage multiple consults and changing priorities in a fast-paced inpatient environment 
Document patient interactions and care plans accurately in Epic EMR 

Required Qualifications 
Active Massachusetts LICSW License 
Master of Social Work (MSW) required 
Epic experience required 
Ability to read, write, and communicate effectively in English 
Strong interpersonal, communication, and negotiation skills 
Excellent organization and time management abilities 
Ability to work independently in a fast-paced, high-pressure environment 
Proficient computer and documentation skills 

Preferred Qualifications 
Acute care hospital social work experience preferred 
CPI (Crisis Prevention Intervention) certification preferred upon hire 
(CPI certification required within 1 month of start date and renewed annually)
