Job Overview The Wellness Nurse is responsible for monitoring the health and well-being of our residents and providing leadership support and assistance with resident care and services. Responsibilities include but are not limited to overall well‑being of our residents, assisting in training and monitoring of medication care administration, communication with resident support services and families, and maintaining clinical quality assurance. Responsibilities & Qualifications Coordination of Health Needs Identify ongoing needs and services of residents Ensure proper follow through with evaluation and documentation Assist Resident Care Director in conducting resident evaluations and data gathering Conduct monthly wellness visits Monitor and follow up with residents after falls, accidents, or incidents Document all pertinent information in resident wellness file Maintain communication with resident families Contact attending physicians as needed Ensure weights and vital signs are obtained monthly Assist in maintaining wellness files according to policies and regulations Maintain medical supplies and emergency kits Provide clinical support and assistance to team members as needed Follow infection control practices and standards Medication Programs Monitor each resident's medication and treatment profile regularly Transcribe physician orders to MAR/TAR Complete MAR/TAR audit each month Demonstrate knowledge of Six (6) Rights of Medication Pass Act as liaison for pharmacy services Monitor medication room and carts for neatness, cleanliness, and expiration Complete medication observations and skills checks for medication care managers Quality Assurance and Regulatory Compliance Demonstrate knowledge of key quality improvement areas: Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes Participate in Sunrise Quality Assurance and Clinical Meetings Knowledge of federal, state/provincial, local regulations, resident rights, and assessment processes Review physician reports of new residents, order medications and equipment as directed Monitor implementation of resident care programs (skin, nutrition, behavior, falls, bowel/bladder) Assist in documentation related to resident's clinical health and wellness Report changes or concerns to Resident Care Director as appropriate Contribute to engagement programs and participate in surveys and improvement workshops Attend regular meetings (Town Hall, Department Team, Cross Over, etc.) Maintain required training and compliance with regulatory authorities Core Competencies Ability to handle multiple priorities Written and verbal communication skills and ability to teach and guide team members Organizational and time management skills Good judgment, problem solving, and decision‑making skills Experience and Qualifications Graduate of approved college/school of nursing Current state/provincial license as Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) Minimum of one (1) year experience in home health, assisted living, hospital, or long‑term care environment Knowledge of nursing practices and federal, state/provincial, local regulations Good assessment skills Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), Sunrise applications, and ability to learn new applications Ability to work weekends, evenings, and flexible hours Benefits Medical, Dental, Vision, Life, and Disability Plans Retirement Savings Plans Employee Assistant Program / Discount Program Paid time off (PTO), sick time, and holiday pay FlexPay offered to get paid within hours of a shift Tuition Reimbursement Pre‑Employment Requirements Employment is contingent upon successful completion of required pre‑employment screenings and health evaluations in accordance with applicable federal, state, and local laws.
