Following a hospital stay, patients may need extended in-hospital care such as physical rehabilitation or specialized neurological care for conditions like stroke, Alzheimer’s, or Parkinson’s. We’re excited to get to know you and learn what you will bring to this specialized nursing role. Responsibilities Provide skilled nursing care for SNF patients in a PRN/day shift setting. Support patients and families with recovery plans and goals; encourage, comfort and motivate throughout the recovery journey. Collaborate with the care team to ensure safe, effective, and compassionate care in the post-hospital setting. Qualifications The Must-Haves Current nursing license to practice in Louisiana as defined by the Louisiana State Board of Nursing. Current American Heart Association BLS certification.
