Coordinate admissions, discharges, and patient care planning with interdisciplinary teams. Monitor clinical documentation, medication administration, and compliance with regulatory standards. Provide staff coaching, mentorship, and performance feedback. Communicate effectively with families, providers, and rehab partners to support optimal recovery. Qualifications Active RN license in the state of New Jersey (required). Previous experience in subacute/rehab or similar setting. Strong leadership, organizational, and communication skills.
