Case Manager Registered Nurse (RN) Days
We are seeking a Case Manager RN to join our team, focusing on patient care coordination and management. This role requires strong communication and multitasking abilities.
Key Responsibilities:
- Coordinate patient care with hospital teams
- Review admissions and plan safe discharges.
- Collaborate on post discharge care and facility transfers.
- Manage 15-20 adult patient cases.
- Perform case management and utilization review.
Schedule:
- 13-week contract, 40 hours/week
- Day shifts: 8:30 am - 4:00 pm
Requirements:
- Active CA RN License
- BLS and LA County Fire Card
- 1 year of case management and 2 years of RN experience
- Inpatient and utilization review experience
- COVID vaccination required
Additional Information:
- No float or on-call duties
- Up to 4 days RTO
- Orientation every Monday
- Local travelers welcome
Work Environment:
- Scrub Color: Royal/Navy Blue
- PPE provided, onsite fit testing
- On-site parking
- EMR: Epic
