Your Territory

Patient opportunities may include Chantilly Centreville Fairfax Fair Oaks Oakton Burke Clifton and surrounding communities.

You do NOT have to cover the entire territory. Tell us where you want to work and well match opportunities based on patient needs and your availability.

What Youll Do

• Complete skilled nursing visits in patients homes

• Perform comprehensive nursing assessments

• Complete Start of Care Recertification Resumption of Care and Discharge visits as assigned

• Provide wound care medication management and disease education

• Monitor changes in patient condition

• Educate patients and caregivers

• Coordinate care with physicians therapists and other members of the care team

• Complete accurate and timely documentation using WellSky

What Were Looking For

• Active Virginia Registered Nurse (RN) License

• Graduate of an accredited nursing program

• Current CPR/BLS Certification

• Valid Drivers License and reliable transportation

• Strong clinical assessment and communication skills

• Ability to work independently in patients homes

• Home Health and OASIS Experience Preferred but Not Required

• WellSky Experience is a Plus
