As a part of our collaborative clinical care team, you will leverage data, technology, and operational rigor to optimize great patient outcomes while offering highly personalized care and shared decision making with patients.This is an in office role, supporting our clinic in Starbase, TX. Our clinic is open Monday through Friday from 8am-4pm CST, with future plans to be open until 8pm CST.Here's what you'll do:Act as a key PCP for our patient population, delivering care in the clinic and digitallyProvide primary care across the full scope of preventive health, chronic disease management, urgent care, and pediatrics.Collaborate and partner across care team members including behavioral health, virtual urgent care and triage, and clinical subspecialistsEducate patients on their health conditions and develop individualized care and treatment plansSolve patient problems in an efficient, nimble manner by drawing on resourcefulness, collaborating with team members to leverage their expertise, and a "can-do" approachAbout YouYou are a Family Medicine Physician with extensive clinical experience. You are comfortable with complex clinical and social needs and eager to care for patients in a modern, technology-enabled practice.
