Job Description

We are seeking a dedicated and compassionate Medical Assistant. This position is ONSITE 5 days a week and 100% Phone based

Pay Rate-

Entry Level: $20 an hour

Experienced (2 years minimum): $22.50 an hour

Responsibilities

• Conduct monthly CCM and RPM patient outreach calls.
• Review medications, treatment plans, and patient concerns.
• Monitor blood pressure readings and respond to RPM alerts.
• Coordinate care between patients, providers, pharmacies, and medical equipment vendors.
• Assist with obtaining and facilitating physician orders for durable medical equipment.
• Document patient interactions and maintain compliance within the CCM/RPM platform.
• Ensure Medicare-required patient engagement time is achieved and documented.
• Educate patients and provide support regarding chronic health conditions.
• Identify barriers to care and escalate concerns to providers when necessary.

Essential Skills

• Medical Assistant Diploma
• Strong interpersonal and communication skills

Work Environment: This position is office-based , working onsite Monday through Friday from 9:00 AM to 5:00 PM in Fresh Meadows, Queens, NY.

This is a Permanent position based out of Fresh Meadows, NY.

The pay range for this position is $41600.00 - $52000.00/yr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.

Full time medical assistant care coordinator role, Monday-Friday 9am-5pm.

This is a Permanent position based out of Fresh Meadows, NY.

This position is anticipated to close on Oct 30, 2026.

This is a fully onsite position in Fresh Meadows,NY.
