Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Family Medicine Physician to join CapitalCare Family Practice Slingerlands , a well-established primary care practice providing comprehensive family medicine services to patients of all ages in a welcoming, team-based environment.

Position Overview

As a Physician at CapitalCare Family Practice Slingerlands, you will provide full-spectrum family medicine care alongside experienced physicians and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and strong patient relationships, with modern tools such as telemedicine and a secure patient portal to support coordinated care and patient access.

Responsibilities

• Conduct comprehensive patient evaluations and manage acute and chronic conditions
• Diagnose illnesses, develop treatment plans, and prescribe medications as appropriate
• Order and interpret diagnostic tests and coordinate specialty referrals as needed
• Provide preventive care, annual physicals, and wellness visits across the lifespan
• Collaborate closely with Advanced Practitioners, nurses, and support staff to ensure continuity of care
• Maintain accurate and timely clinical documentation in accordance with organizational standards

Requirements

• Qualified candidates should have completion of Doctor of Osteopathy (DO) or Doctor of Medicine (MD) , completion of an accredited internship and residency and an active license to practice medicine in NYS.
• Must be currently board certified by an American Board of Medical Specialties (ABMS) Member Board or an approved American Osteopathic Association (AOA) specialty board.
• Upcoming new graduates will still be considered in a board eligible status.
