- Provide high-quality physical therapy services to patients in a home or outpatient setting.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Conduct patient assessments, track progress, and adjust treatment plans as needed.
- Focus on rehabilitation, recovery, and improving patient mobility and quality of life.
- Participate in team meetings and communicate effectively with colleagues to ensure continuity of care.
- Maintain accurate and timely patient documentation.
- Valid Physical Therapist License (required).
- Basic Life Support (BLS) Certification (required).
- Minimum of 1 year of experience in physical therapy, rehabilitation, or related patient care.
- Strong organizational skills and ability to manage caseload efficiently.
- Excellent interpersonal and communication skills.
- Commitment to patient-centered care and continuous professional growth.
