This position is the Health Department Officer for Jackson County Kansas and involves providing high-quality nursing care to patients from various settings of community health. The ideal candidate will possess a diverse skill set and experience in multiple areas of nursing, ensuring comprehensive patient assessment and management. Responsibilities Conduct thorough patient assessments and develop individualized care plans for the WIC program. Administer immunizations and treatments as prescribed. Collaborate with co-health departments on services offered to communities. Assist with the growth of services offered by the health department Collaborate with local schools and other healthcare entities in area for community needs assessments. Perform health education trainings for the community to include schools, senior centers and health department. Collaborate with interdisciplinary teams to provide holistic care for patients with chronic conditions such as diabetes. Maintain accurate medical documentation in compliance with HIPAA regulations and NCQA standards. Utilize electronic health record (EHR) systems such as Cure for patient management. Educate patients and their families on health management, medication administration, and disease prevention strategies. Participate in triage and emergency response and disaster preparedness situations as needed. Collaborating with the state of Kansas on investigations for epidemiology cases located in the county. Experience Candidates should have a strong background in nursing with experience in various healthcare settings. Preferred qualifications include: Experience with teaching patients about their health needs Proficiency in phlebotomy, specimen collection/processing, and diagnostic evaluations Knowledge of medical terminology, anatomy, physiology, and infection control practices Experience with EMR systems. Background in community health nursing is preferred but not required.
