Hospice Home Health Registered Nurse - Vienna, WV

Deliver exceptional, compassionate hospice care as a Registered Nurse, supporting patients and families facing end-of-life transitions in homes and residential settings. This contract position emphasizes a patient-centered approach, ensuring comfort and dignity while collaborating closely with an interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotation may be required

Key Responsibilities:

• Complete thorough nursing assessments and develop individualized hospice care plans
• Deliver skilled nursing interventions for symptom management, pain control, and patient comfort
• Administer medications and treatments per physician orders and hospice protocols
• Monitor patient changes and promptly update the medical director and care team
• Educate families and caregivers on disease progression, comfort measures, and emotional support
• Collaborate with social workers, chaplains, aides, and volunteers to meet physical, emotional, and spiritual needs
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Engage in case conferences, team meetings, and ongoing quality improvement initiatives
• Ensure full compliance with hospice policies, procedures, and regulatory standards

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year recent hospice experience required
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Strong clinical assessment and interpersonal communication abilities
• Solid foundation in pain management and end-of-life care principles
• Reliable transportation, valid drivers license, and auto insurance
• Independent, adaptable, and compassionate professional demeanor

Preferred Qualifications:

• BSN degree
• Background in palliative or end-of-life care
• Familiarity with Medicare hospice documentation requirements
