GRANT SUMMARY The goal of the Alaskan Outreach and Resiliency (AOR) Project is to improve the health and wellness by preventing the onset of psychosis (CHR-P) and lessen the severity of psychotic disorders among American Indian and Alaska Native (AI/AN) youth and young adults. This project will provide trauma-informed, culturally resonant, evidence-based interventions using a stepped-care model and integrates medication management, psychotherapy, case management, family support, and educational and vocational support tailored to the individual needs of young people experiencing early symptoms of psychosis to improve outcomes and promote recovery. POSITION SUMMARY This position guides participants in goal setting, skill development, and accommodations related to education and employment using the Individual Placement and Support (IPS) Model. This position will provide ongoing support for school or job placement and provide linkages between community agencies, schools, and employers. This position is a member of the AOR clinical team and works with the program participant and their family to achieve the individualized treatment plan. JOB DUTIES

- Develops individualized IPS plans that include support services, working with participants and the treatment team to promote overall health and wellbeing.
- Guides goal setting, skill development, and accommodates related to education and employment, using evidence-based approach Individual Placement and Support model.
- Facilitates access to services, coordinates care among different providers and community resources ensuring a seamless continuum of care for consumers.
- Conducts Screening, Brief Intervention, and Referral to Treatment (SBIRT) with youth referred to
AOR using empathy and engendering optimistic empowerment for consumers, using evidence-based practices identified by FNA. - Is a member of the clinical team and coordinates services within the BHS Service Unit.
- Works with consumers to understand and address risk factors and connects consumers with resources and support services.
- Participates in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvement.
- Ensures adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR).
- Develops individualized educational plans and coordinates tutoring assistance.
- Integrates vocational, education, and mental health services.
- Links consumers with career counseling, job training, mentorship, and internships.
- Promotes and sustains youth & family participation in support services, including peer support, substance use disorder support, youth leadership, mentoring, family counseling that includes acceptance of LGBTQI+ youth.
- Acts as team liaison with outside educators and employers.
- Provides ongoing client support following job or school placement.
- Works with consumers and families to develop individualized education and employment plans.
- Contacts consumers for follow up post-treatment.
- Provides culturally relevant and trauma informed service.
- Attends mandatory position training that provides training in risk factors.
- Enters client information into electronic health record; develops billing records.
- Performs other job-related duties as assigned.
- Ability to report to work in a timely manner.
- Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders.
- Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles.
- Skill in operating/using computers and using Windows, Word, and Excel with ability to accurately input information into reporting formats.
- Above average communication and people skills with the ability to accurately follow written and verbal instructions.
- Knowledge on how to best assist possible escalated individuals during data collection process.
- Ability to protect confidentiality is essential; HIPAA & 42 CFR compliant.
- Knowledge and ability to interpret/understand billing procedures.
- Skill in managing multiple priorities and tasks concurrently and meeting their deadlines.
- Skill in establishing and maintaining cooperative working relationships with colleagues, supervisors, agencies, and consumers.
- Ability to work independently as well as with teams.
- Bachelor's degree in behavioral health or related field.
- One (1) year of experience working with AN/AI population and experience providing culturally specific services.
- Two (2) years of experience involving case management and/or supported employment.
- Two (2) years of experience working with young people and families.
- A combination of experience, education, and training that provides the required knowledge, skills, and abilities may substitute for degree requirements.
- Master's degree in behavioral health or related field.
- Experience with workforce development, IPS Model or working with individuals at clinical high risk for psychosis.
