Nurse Practitioner or Physician Assistant - Outpatient Primary Care

Company Allegheny Health Network Job Description GENERAL OVERVIEW: In addition to base compensation, this role offers annual quality-based bonus tied to contributions to population health initiatives and value-based metrics, rewarding dedication to a team-based approach, improving patient outcomes, and driving efficient high-quality care. This position is integral to the care delivery system, working together to provide a healing environment and placing customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice, and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required. Schedule: Monday AM/PM/EVENING, Friday AM/PM/EVENING, Saturday 8 am‑2 pm. ESSENTIAL RESPONSIBILITIES Provides appropriate patient care: recording medical history and physical assessment, developing a therapeutic care plan, ordering diagnostic tests, interpreting results, and maintaining accurate medical record documentation (25%). Communicates and coordinates patient care with patient, family, community health services, and interdisciplinary team. Involves the attending physician regarding the plan of care and clinical decision making for issues outside the CRNP’s knowledge base or scope of practice (15%). Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement (10%). Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds, and provides wound care (10%). Educates patients and families regarding health and preventive medicine, prescribed medications, treatment instructions, and recommends community resources as appropriate (10%). Triages patient telephone calls, provides monitoring and continuity of care (10%). Participates in, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies (10%). May perform other duties such as managing outpatient concerns and duties specific to the specialty (10%). Other duties as assigned or requested. QUALIFICATIONS Required (CRNP) Master’s degree MSN from a state board of nursing approved Master’s program. National Board Certification for Nurse Practitioners through AANP, ANCC, NCC, or PNCB. Practicing-state licensure as a Nurse Practitioner. Prescriptive Authority from the practicing state within 60 days of hire. Drug Enforcement Agency licensure within 60 days of hire. CPR – American Heart Association. Act 34 Criminal Background Clearance Certificate. Act 33 Child Abuse Clearance Certificate. Act 73 FBI Fingerprinting Criminal Background Clearance Certificate. Preferred (CRNP) None Required (PA) Master’s degree in Physician Assistant Studies. Current PA Physician Assistant License. PA‑C or board‑eligible. Ongoing successful recertification (NCCPA) as a PA‑C within 90 days of hire. Drug Enforcement Agency Licensure within 60 days of hire. CPR – American Heart Association. Act 33 Child Abuse Clearance Certificate. Act 34 Criminal Background Clearance Certificate. Act 73 FBI Fingerprinting Criminal Background Clearance Certificate. Substitutions Four to ten years of related experience and/or training; or equivalent combination of education and experience.
