Job Description Medical Nurse Practitioner- Skilled Nursing Facility Float – Full Time - covering the Hartford/ New Haven/ Fairfield, CT areas. Hiring multiple candidates with SNF experience. Impact Health is an independent practitioner group servicing skilled nursing facilities throughout northern New England. Our Nurse Practitioners are expected to provide the highest level of clinical support, consultation and treatment in collaboration with physicians, nurses and specialists. Our mission is to provide a "whole" treatment plan that is patient-centered with a comprehensive team approach designed to make a difference in the lives of our most vulnerable population. If you have experience in skilled nursing, hospital emergency department, SICU/ICU, long term rehab or like experience diagnosing, treating, prescribing and billing, this role is a perfect fit.
