Key Responsibilities\n

• Work in collaboration with a diverse rehabilitation team to create and implement individualized treatment plans that consider each patient's unique psychosocial needs.

• Assist patients in navigating and accessing essential community resources, such as support networks, vocational training programs, or permanent housing assistance.

• Educate families and patients about available community resources and post-rehabilitation options to enhance their ongoing support.

• Play an active role in discharge planning and facilitate smooth transitions for patients returning home or moving to alternative care settings.

Qualifications\n

Compensation and Benefits\n
