Hybrid RN Case Manager - Home Care

Workstream

Workstream is seeking a motivated Registered Nurse Case Manager for a hybrid role in Auburn, NY. This position involves managing care for clients and supervising Personal Care Aides. The RN will conduct evaluations, ensure quality care, and assist with agency trainings. This role requires a valid RN license and flexibility in scheduling.
