Responsibilities

• Perform initial evaluations and ongoing reassessments for residents requiring physical therapy services.
• Develop, implement, and adjust individualized treatment plans to support mobility, strength, balance, and functional independence.
• Deliver direct PT interventions, including therapeutic exercise, gait training, manual therapy, and neurorehabilitation as appropriate.
• Monitor patient progress, document outcomes, and communicate changes to the interdisciplinary team.
• Collaborate with nursing, therapy colleagues, and family/caregivers to coordinate comprehensive care and discharge planning.
• Ensure patient safety, infection control, and adherence to professional standards and facility policies.
