In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Pueblo, CO

Job Overview Signify Health, part of CVS Health®, is recruiting Part‑time Clinicians (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. Clinicians will visit patients in their homes, build trusted relationships, and deliver comprehensive, personalized health assessments. Job Highlights Each visit lasts approximately 45 minutes, including medication & medical history review, physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry). Clinicians use an iPad and an intuitive clinical workflow. Clinicians are not required to prescribe medication, order lab tests, or alter current treatment regimens. Responsibilities Bring your heart into every visit, joining a national network of purpose‑driven clinicians. Visit millions of people nationwide to connect them to the right care for their unique needs. Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role. Travel locally and within licensed states as needed. Conduct virtual visits as required. Board certification (required). Multi‑state licenses (preferred). Able to cover primary locations: Fremont, Saguache, Custer, Alamosa, Costilla, Huerfano, Pueblo, Crowley, Otero, Las Animas. Pay & Hours Anticipated weekly hours: 29.5. Pay range: $41.85 to $90.13 per hour (base hourly rate or base annual full‑time salary).
