This outpatient PACE (Program of All-Inclusive Care for the Elderly) clinic is dedicated to providing comprehensive, person-centered care for elderly and frail participants in the San Diego area. The facility operates with a strong interdisciplinary team (IDT) model, fostering a collaborative and supportive work environment focused on delivering high-quality home-based care coordination services.

PACE Home Care Coordinator – Outpatient Clinic
Job Details:
- Serve as a collaborative member of the Interdisciplinary Team (IDT), providing foundational support to the Home Care Coordination team under the supervision of the PACE Home Care Coordination RN Manager
- Conduct pre-enrollment, annual, semi-annual, and change of address home safety evaluations for PACE participants
- Coordinate IDT-approved services including meal delivery, durable medical equipment (DME), incontinence supplies, and caregiver services to participant homes
- Complete the Home Care Coordination care plan at enrollment and semi-annually thereafter as a member of the IDT
- Document and track actions, services, and service delivery status updates in designated logs and participant electronic medical records (EMR) in compliance with DHCS and CMS regulations
- Review progress notes and documentation of service delivery, informing the IDT of any gaps in service for reconciliation
- Upload progress notes, reports, and care/vendor-related documentation to the EMR
- Communicate regularly with participants, their families, and other healthcare providers to ensure coordinated, person-centered care delivery
- Advocate for participants' healthcare needs and preferences, ensuring compassionate and person-centered care
- Collaborate with interdisciplinary team members including physicians, social workers, therapists, and other healthcare professionals
- Conduct and document monthly visits for homebound and RCFE participants, as well as home visits as needed
- Assist PACE nurses in the coordination and delivery of meals, DME, caregiver services, and other support as requested
- Deliver supplies, equipment, grocery bags, and other items as requested or needed
- Generate and maintain reports of services provided for participants
- Assist participants with surveys and participate in interdisciplinary team meetings as needed
- Adhere to infectious disease precautions and all safety standards; actively participate in the Safety and Injury Prevention in the Workplace Program
- Attend center staff meetings and in-service educational training programs
- Must possess a valid CNA certification and California Department of Public Health Home Health Aide (HHA) certificate
- Must hold current CPR and First Aid certification from the American Heart Association
- Minimum one year of experience working with a frail or elderly population required
- Valid California Driver's License required
- Basic computer skills required including Microsoft Office, Outlook, and Excel
- Schedule: Monday – Friday, 8:00am – 5:00pm
