Social Worker 2 - Adult ER: 3rd Shift (260004EO) The Social Worker 2 role within the UH Cleveland Medical Center – Adult Emergency Room offers a unique chance to be part of a dynamic, collaborative interdisciplinary team that enhances patient care and assists individuals with a variety of psychosocial needs. Benefits Free parking for regular status caregivers Full benefits and retirement Support for continuing education, career growth, and professional development Responsibilities Provides psychosocial assessment/intervention to assist patients and families, including clinically complex cases Provides age‑specific care to patients and families across all age groups Develops, documents, and implements a comprehensive plan based on identified psychosocial and discharge needs Develops plans with attention to individual patient and family goals Links patients to community resources and UH network services to facilitate optimal care, serving as liaison to community agencies Communicates and collaborates with the work team using a multidisciplinary approach Offers guidance and acts as a consultant for staff and other disciplines regarding complex psychosocial and/or discharge planning issues and barriers Assists with recruitment, orientation, mentoring, and education of new staff and/or social work interns Additional Responsibilities Performs other duties as assigned Complies with all policies and standards Refer to documentation provided by the department during orientation for specific duties and responsibilities Abides by all requirements for protecting and securely maintaining PHI, including annual training and adherence to the UH Code of Conduct and policies Education Master’s Degree from an accredited school of Social Work (Required) Work Experience Hospital or health agency experience (Preferred) Knowledge, Skills, & Abilities Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Ability to multi‑task and prioritize work (Required proficiency) Works autonomously (Required proficiency) Communicates effectively with persons of various backgrounds (Required proficiency) Licenses and Certifications Licensed Social Worker (LSW) – Required Licensed Independent Social Worker (LISW) – Preferred within one year of eligibility Previous social work experience is strongly preferred. Prior work in an emergency room or urgent care is also preferred. Schedule: Monday‑Friday with rotating weekends and holidays. This position is full‑time, 3rd shift (8:00 pm start time) in the Adult Emergency Department.
