Position SummaryThe Nurse Trainer plays a vital role in promoting the health, safety, and well-being of individuals receiving services by ensuring high-quality medical oversight and safe medication practices in staffed and shared community residences. This position provides clinical consultation and support to staff, providers, and families, monitors healthcare coordination, and ensures compliance with New Hampshire He-M 1201 regulations and the 404 Nursing Delegation Rule. The Nurse Trainer partners with interdisciplinary teams to enhance health outcomes through education, quality assurance, and person-centered care.
PAY: $80,000/yr.
Essential Responsibilities Medication Management & Clinical Consultation- Provide medication consultation and clinical guidance to staff, providers, and families for individuals on the assigned caseload.
- Maintain accurate and up-to-date medical records, including medication lists and physical examination documentation.
- Educate staff and providers on medications, side effects, and related health concerns.
- Communicate medication-related issues and recommendations to Residential Managers and other appropriate team members.
- Complete documentation in accordance with agency policies and regulatory requirements.
- Attend interdisciplinary team meetings as needed.
- Collaborate with the Nursing Team and Case Managers to develop individualized health protocols for individuals with high medical needs.
- Identify training requirements and provide education related to specialized medical care, including:
- G-tube care
- Seizure management
- Cast care
- Other specialized medical conditions and procedures
- Ensure compliance with New Hampshire He-M 1201 regulations and the 404 Nursing Delegation Rule.
- Conduct medication quality assurance reviews:
- Every 1–6 months for family home care providers.
- Monthly for staffed residences and day programs.
- Distribute completed quality assurance reviews to program leadership and maintain nursing records.
- Assist programs and vendors in obtaining medication waivers when appropriate.
- Observe medication administration and ensure annual medication certification for assigned staff and providers.
- Assess individuals' ability to safely self-administer medications in accordance with He-M 1201 regulations.
- Develop medication training plans when appropriate.
- Complete and distribute assessment documentation to appropriate providers, managers, case managers, and guardians when applicable.
- Monitor individuals' medical, dental, and behavioral health care to promote comprehensive wellness.
- Serve as a liaison with physicians, hospitals, and mental health providers as needed.
- Notify Case Managers and Dental Coordinators of identified healthcare needs.
- Coordinate diabetes education and training with qualified instructors for staff supporting individuals with diabetes.
- Investigate, document, monitor, and evaluate medication occurrences.
- Review medication occurrences with staff and providers to identify opportunities for improvement.
- Collaborate with supervisors and coordinators on follow-up actions as needed.
- Complete required Medication Error Reports monthly.
- Complete Medication Action Reports every six months as required by He-M 1201.
- Provide basic first aid and nursing consultation to staff and individuals during emergency situations as appropriate.
- Participate in Nursing Department meetings and other assigned meetings.
- Attend Individual Service Agreement (ISA) meetings when appropriate.
- Actively contribute as a collaborative member of the Nursing Department.
- Demonstrate teamwork and maintain regular attendance, including in-office and face-to-face interaction, as essential functions of the role.
- Perform additional duties as assigned.
- Current New Hampshire Registered Nurse (RN) license in good standing.
- Minimum of three years of nursing experience, preferably in public health, community-based services, or supporting individuals with intellectual and developmental disabilities.
- He-M 1201 Nurse Trainer Certification, or the ability to obtain certification within three months of employment.
- Valid New Hampshire driver's license and reliable transportation available during work hours.
- Proficiency with Microsoft Office Suite and electronic database systems.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Strong commitment to person-centered care and exceptional customer service for individuals, families, and colleagues.
- Excellent organizational, time management, and prioritization skills.
- Strong critical thinking, problem-solving, and decision-making abilities.
- Effective verbal and written communication skills.
- Ability to educate, motivate, and support staff and providers through coaching and training.
- Comfortable working in a variety of community-based settings.
- Flexibility to work occasional early mornings, evenings, or adjusted schedules based on operational needs.
- Ability to participate in the on-call rotation approximately one week per month.
- Demonstrated ability to work effectively with individuals from diverse educational, cultural, and socioeconomic backgrounds.
- Proficiency in Microsoft Word, Excel, Outlook, and the ability to learn new software systems.
