- Monday - Friday 8:00AM - 4:30 PM
- Perform comprehensive nutrition screenings and assessments.
- Develop and implement individualized Medical
- Nutrition Therapy(MNT) plans.
- Document nutrition assessments, interventions, and outcomes within the electronic medical record.
- Provide nutrition groups, education and counseling to patients, families, and caregivers. Participate in facility-wide and
- departmental Quality Assurance and Performance Improvement initiatives.
- Participate in multidisciplinary patient care rounds and departmental meetings.
- Support staff development initiatives and contribute to improved clinical outcomes.
- Complete Swallowing Screen as needed.
- Current, valid, and unrestricted NY license/certification in good standing, as required for the position.
- Bachelor's or Master's Degree in Clinical Nutrition, Dietetics or a related field from an accredited institution
- Registered Dietician(RD) or Registered Dietitian Nutritionist Credential.
- Minimum of one (1) year related experience.
- Additional certifications may be required.
- Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
- Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test, or other client-specific requirements ) .
