Typical caseloads are managed to allow for meaningful one-on-one time in the home, with visit lengths designed to support thorough assessment, intervention, and patient and caregiver education. Patient populations commonly include older adults recovering from joint replacements or fractures, individuals managing cardiac or pulmonary conditions, patients after hospital or skilled nursing facility discharge, and those living with neurologic conditions such as stroke or Parkinson's disease.
