American Traveler is seeking a Physical Therapist for a Home Health assignment, requiring at least 1 year of home health PT experience and a valid driver's license.
Details
- Home health setting covering the Mokena/Frankfort territory
- Day shift, 8-hour shifts
- Provides care to patients ranging from pediatric to geriatric populations
- Mileage reimbursed at standard IRS rate
Requirements
- Minimum 1 year of home health PT experience required
- Valid driver's license and car insurance required
- BLS certification required
- IL state license required if applicable
- OASIS and start of care experience strongly preferred
- COVID vaccination required with no declinations approved
- Must document vaccination status in profile
Additional Information
- Responsibilities include evaluating, planning, treating, and implementing individualized plans of care for home health patients
- Communicates effectively with patients, families, physicians, and staff in alignment with hospital core values
- Completes timely documentation of all patient care activities in the electronic medical record
- Must be able to transfer and position patients
- Local candidates considered if within reasonable distance of the territory
- Maximum of 5 days RTO may be requested during the assignment
